Ballet is the word connected to an image of a couple of people, one male and one female dancing smoothly and flexibly with classic music. Dance allow me to imagine not a picture but the sentiment that someone feels when he or she dance; the freedome of movement and the authenticity of each individual.
The pictures which uses to come to my mind is the one of a male spanish dancer of ballet, called Nacho Duato. There is a very famous picture of him dancing and it is really amazing how he looks. Upon hearing dance... I see myself dancing with my friends in a disco!
Upon hearing the word "ballet", i think this is the base for styding art and i think of a dancer when he/she was young.. Dance for me is an oriental dance, a latin dance, flamenco, even zeibekikos and r&b Dance.. I think of many dancers dancing the various forms i whote earlier..and they are flexible, smiling and look and feel happy..(hope i was helpful enought, good luck!)
Ballet gives me the picture of gorgeous ballerinas, the opera stage, the orchestra and all that. It also see a dance-style pretending things are what they aren't. Internal struggle and external ease.
Dance gives me the picture of moderen dance in general. A picture of real emotions. Expressions of the actual struggle it is to be a dancer, inviting the audience to in to join. Freedom.

Upon hearing the word "ballet", Degas' paintings of ballet dancers come to my mind.
Upon hearing dance...well, it depends of my state of mind!

Hearing "dance", I see a night club and lots of people shaking.
